We are seeking a compassionate and motivated Lead General Practice Nurse to deliver high-quality, patient-centred care. The role involves assessing patient needs, supporting treatment delivery, monitoring progress, and working collaboratively with the wider clinical team to ensure safe and effective care. The role will include leading an already established team and support the Practice Manager and Clinical CQC Manager to ensure the practice is up to date with all processes and procedures.

Main duties of the job
The role involves delivering high-quality nursing care within a GP practice, including patient assessment, chronic disease management, cervical cytology and immunisations. The nurse will support infection control standards, perform clinical procedures such as wound care and ECGs, and contribute to hypertension management. Responsibilities also include supporting vaccination programmes (including flu campaigns), maintaining accurate clinical systems (e.g. EMIS), participating in audits and QoF targets, and working collaboratively with the wider clinical team. The role requires keeping up to date with NICE guidance and contributing to the development and review of nursing protocols to ensure safe, effective patient care.

The following are the core responsibilities of the Practice Nurse in delivering health services. There may be, on occasion, be a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

Support and on occasion lead with Infection Prevention Control (IPC) and work to the IPC policy
Consult with patients and their families to undertake assessments of patient need
Management of chronic disease
Cervical Cytology
Childhood Immunisation
Administer a range of adult vaccinations
Organise and support Practice Manager with the flu campaign
Wound care
ECGs
Hypertension management
Actively participate in achieving QoF and enhanced services targets and recall demands
Understand EMIS and EMIS searches
Participate in practice audit as directed by the Clinical Lead/Practice Manager
Support and participate in shared learning within the practice
To be an instrumental part of the clinical team attend regular nurse meetings
Keep up to date with NICE guidelines as required
Develop, review and implement nursing protocols in conjunction with the partners
