We are seeking a motivated nurse with experience in cervical screening, immunisations, chronic disease management and venepuncture to help us deliver high-quality care to our patients across our two Harpurhey sites.

Main duties of the job
The successful candidate will provide high-quality nursing care to our patient population, working as part of a multidisciplinary primary care team across our two Harpurhey sites.
Key responsibilities will include:
Performing cervical screening (smear tests) in line with national guidelines.
Delivering childhood, adult and seasonal immunisations and vaccinations.Supporting the management and monitoring of patients with long-term conditions, including chronic disease reviews and health promotion.Undertaking venepuncture and other routine clinical procedures.Providing evidence-based advice and education to patients to support self-management and wellbeing.Maintaining accurate clinical records and ensuring care is delivered in accordance with practice protocols and professional standards.Working collaboratively with GPs and the wider multidisciplinary team to deliver safe, effective and patient-centred care.The role offers the opportunity to make a significant contribution to patient care while working within a supportive and welcoming practice team.

Clinical Duties
Undertake cervical cytology (smear) screening in accordance with national and local guidelines.
Administer childhood, adult, travel, seasonal influenza, COVID-19, and other routine immunisations and vaccinations.
Carry out venepuncture procedures safely and effectively.
Provide chronic disease management clinics, including monitoring and supporting patients with conditions such as:
Diabetes
Asthma
COPD
Hypertension
Cardiovascular disease
Perform health assessments, lifestyle advice, and health promotion activities.
Undertake wound care, dressings, and other treatment room duties.
Maintain accurate clinical records using EMIS Web.
Process and action clinical tasks, recalls, and patient communications within EMIS.
Participate in infection prevention and control activities and ensure compliance with practice policies.
Support audit, quality improvement, and QOF activities.
Administration and Record Keeping
Maintain accurate, contemporaneous electronic patient records.
Use EMIS Web effectively for clinical documentation, recalls, and chronic disease monitoring.
Contribute to data quality initiatives and service improvement projects.
Ensure compliance with information governance, confidentiality, and GDPR requirements.
Person Specification
Experience Essential
Registered Nurse with current NMC registration. Experience of working within a primary care or nursing team. Experience of dealing with vulnerable patients and those with complex health needs. Competent in cervical cytology (smear taking). Experience of administering routine childhood and adult immunisations and vaccinations. Competent in venepuncture. Experience of managing patients with long-term conditions, including diabetes, asthma, COPD, and hypertension. Ability to work independently and prioritise workload effectively. Excellent communication and interpersonal skills. Ability to maintain accurate and contemporaneous clinical records. Understanding of safeguarding responsibilities for children and vulnerable adults. Knowledge of infection prevention and control standards. Ability to work collaboratively within a multidisciplinary team. Commitment to delivering high-quality, patient-centred care. Good IT skills, including the use of clinical computer systems. Understanding of confidentiality, GDPR, and Information Governance requirements.
Desirable
Previous experience working as a Practice Nurse in General Practice. Experience of using EMIS Web. Chronic Disease Management qualifications or relevant training. Independent Prescriber qualification. Travel Health qualification. Experience of participating in clinical audit and quality improvement activities. Mentorship or teaching experience. Knowledge of QOF and Enhanced Services requirements. Current Basic Life Support and Immunisation Update training. Experience of health promotion and lifestyle intervention programmes.
Qualifications Essential
Registered Nurse (NMC Registered). Qualified Cervical Smear Taker. Experience in administering immunisations and vaccinations. Competent in venepuncture. Experience in chronic disease management within primary care. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of clinical governance and safeguarding requirements.
Desirable
Experience working in General Practice. Knowledge and experience of EMIS Web. Chronic disease management qualification(s). Independent Prescriber qualification. Travel health qualification.
