The Deerbrook Surgery is seeking an enthusiastic, experienced, and patient-focused Practice Nurse to join our friendly and supportive primary care team for 30 hours per week.

We are looking for a motivated nurse with previous experience in general practice who is committed to delivering high-quality, evidence-based care to our patients. The successful candidate will be confident working both independently and as part of a multidisciplinary team, with excellent clinical and communication skills.

Main duties of the job

Chronic disease management (including asthma, COPD, diabetes, and hypertension) Cervical screening Childhood and adult immunisations Travel vaccinations (where appropriate) Wound care and dressings Ear irrigation (where trained) Health promotion and lifestyle advice NHS Health Checks Venepuncture Blood pressure monitoring Contraceptive advice and administration Assisting with the management of long-term conditions Supporting the delivery of QOF and enhanced services Accurate clinical documentation using the practice clinical system Working collaboratively with GPs, healthcare assistants, pharmacists, and the wider practice teamInfection Prevention and Control

Ensuring compliance with national infection prevention and control guidance. Supporting the practice's infection control policies and procedures. Undertaking infection control audits and contributing to quality improvement. Promoting safe clinical practice and maintaining a clean and safe clinical environment.Essential Criteria

Registered Nurse with current NMC registration. Previous experience working as a Practice Nurse within General Practice. Experience in chronic disease management. Competent in immunisations and cervical cytology. Sound knowledge of infection prevention and control. Excellent clinical assessment and organisational skills. Strong communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Good IT skills and experience using GP clinical systems.

To provide high-quality nursing care to patients registered with the practice, working autonomously and collaboratively with the wider healthcare team. The Practice Nurse will support the delivery of excellent patient care through health promotion, disease prevention, chronic disease management, and clinical assessments in line with professional standards and practice policies.

Person Specification

Qualifications Essential

Registered Nurse with current registration with the Nursing and Midwifery Council. Recognised nursing qualification (e.g., Diploma in Nursing or Degree in Nursing). Evidence of continuing professional development (CPD) relevant to primary care nursing. Experience and competence in clinical assessment and patient care. Knowledge and understanding of current primary care nursing standards and guidelines. Basic Life Support (BLS) training (current or willingness to complete). Evidence of mandatory training compliance, including: Infection prevention and control Safeguarding adults and children Health and safety Equality, diversity, and inclusion Information governance/confidentiality

Desirable

Primary Care Practice Nursing qualification or equivalent experience. Mentorship or teaching qualification. Independent prescribing qualification (or willingness to undertake training). Cervical cytology/screening qualification. Immunisation and vaccination training. Long-term condition management qualifications (e.g., diabetes, asthma, COPD, cardiovascular disease). Travel health qualification. Minor illness or assessment and diagnostic skills training.
