Job summary
Hyde Park Surgery is looking for a clinical data administrator to join our team to assist in the delivery of the highest standard of care. This is a varied role working with clinicians and managers to help develop, implement and maintain processes to achieve excellent outcomes for our patients. The successful candidate will need to have good IT skills, have an eye for detail, be pro-active and enthusiastic. You will work with and be supported by clinicians and admin staff and form part of our team providing care for our diverse patient populations. Please see the job description for more details.
Main duties of the job
MAJOR DUTIES AND RESPONSIBILITIES:
Clinical coding:
Entering accurate information onto patient records.
Arrange any actions as appropriate
Administer the call and recall system:
Ensuring patients are invited to all appropriate reviews.
Ensure a safety net process is in place.
Develop new systems for recall as technology changes.
Quality Schemes (QOF & GPOP etc.)
Review the schemes on an annual basis and advise of changes and plan methods for achievement.
Run regular patient searches and report to practice teams to support achievement.
Summarise patient records
Enter relevant medical information on new patients electronic medical records as per the summarising protocol.
Checking files sent electronically through GP2GP are correct.
Ensuring the summarising protocol is kept up to date.
Document Management
Assist with workflow duties.
Ensure actions resulting from incoming workflow is completed.
Any other delegated duties considered appropriate to the post. An ability to adapt to new circumstances will be essential.
About us
Hyde Park Surgery is situated in an inner-city area within the Woodsley Primary Care Network (PCN) under Leeds Integrated Care Board (ICB).
There are four partners in the practice. We also have three salaried GPs. Our nursing team consists of two practice nurses and two health care assistants. The team provides a holistic and collaborative approach to health care provisions within the practice. Our Practice Manager works full time and is supported by the Assistant Practice Manager. We have a hard working and dedicated team of receptionists who are supported by the Reception Manager.
The practice population includes a significant proportion of ethnic minority patients, mainly of South Asian origin. We have a high young and lower elderly population as many students living within the practice boundary register with the practice.
We provide a full range of primary care services including maternity care, child health surveillance, contraceptive services, minor surgery and long term conditions clinics.

MAIN PURPOSE OF THE POST:Medical data coding from clinical correspondence to ensure accurate and timely updating of medical records. Administer the call and recall system. Running patient and reporting data to support the Quality and Outcomes Framework (QOF) and all Quality Schemes. Summarise patient records and GP2GP transfers and assist with document workflow and provide administrative support to the practice.
RESPONSIBLE TO: Management Team
HOURS PER WEEK: 22.5 hours per week (negotiable for the right candidate)
SALARY: £15.50 per hour (depending on experience)
SPECIAL REQUIREMENTS OF THE POST:
An understanding, acceptance and adherence to the need for strict confidentiality.
Excellent knowledge of medical and pharmaceutical terms.
Understanding of the summarising process and adherence to the practice summarising style.
Excellent attention to detail and accuracy to spot errors and anomalies in a record, and to enter medical information correctly.
Ability to read and process information quickly.
Good knowledge of SystmOne clinical system and general computer literacy.
A methodical and logical approach to work.
The ability to use own judgement, resourcefulness and common sense.
A genuine care about the quality of patient data and improving patient outcomes through accurate coding and the maintenance of medical records.
Willingness to comply with all mandatory training including smart card and Information Governance training.

Person Specification
Qualifications Essential
Knowledge of SystmOne clinical computer system. Computer literate and experience of clinical coding and summarising.
Experience Essential
Experience of working in primary care.
