Job summary

We are a friendly and innovative training practice, which is committed to high quality care and the continued development of our team. We are looking for a dedicated, enthusiastic, and motivated Practice Nurse to join us. Experience in management of Chronic disease, Anti-coagulation, Cytology and Baby Immunisation would be desirable.

The salary is negotiable and will be dependent on qualifications and experience.

You will be fully supported by our GPs & practice management team and as a patient-focused practice, in-turn you will be asked to deliver the highest quality service to our patients and our practice community. The successful applicant will be a dynamic individual, able to evidence qualifications and experience in a wide range of medical disciplines within primary care and general practice. The successful applicant will also need to evidence knowledge of QAIF and chronic disease management.

We are actively involved in the Blaenau Gwent West NCN Cluster.

7000+ patients, 3 GP Partners, 1 Long-Term Full-Time Locum, 1NP, 1HCA all complimented with an excellent administration team.

PRACTICE NURSE (35 – 37.5 hrs pw) wages negotiable by

ORGANISATIONAL ARRANGEMENTS

Responsible to: General Practitioners, Practice Manager

Accountable to: Professionally - Patients under NMC Code of conduct

Supervision by: General Practitioners, Practice Manager, Nurse Practitioner

JOB PURPOSE:

The post holder is responsible for the delivery of safe and effective nursing care to the whole practice population, being accountable for nursing service delivery. To work with the practice team and GP’s to deliver the practice priorities. The focus of the role is the delivery of evidence-based practice for patients and the provision of preventative health care to the practice population. The nurse is responsible for the care delivered, demonstrating skills in clinical decision-making. The post holder will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership and direction as required. The role is held by one who is a selfdirected practitioner, highly motivated, flexible, enthusiastic and works willingly as a team member and also able to diversify across boundaries when required.

SCOPE

• To deliver a high standard of patient care as a practice nurse in general practice, using

clinical skills and a broad and in-depth theoretical knowledge obtained through specialise

education, experience, courses and updates.

• To assess plan and implement treatment programmes that promote health

• To support other nurses in developing and maintaining clinical skills

PRIMARY DUTIES AND AREAS OF RESPONSIBILITY

CLINICAL ROLE

The post holder will:

• Offer a holistic service to patients and their families, developing where appropriate an

ongoing plan of care and support with an emphasis on prevention and self-care

• To diagnose and manage both acute and chronic conditions, integrating both drug and

non-drug-based treatment methods into a patient management plan

• Prioritise health problems and be able to initiate effective emergency care

• Promote health and wellbeing through the use of health promotion, health education,

screening and therapeutic communication skills

• Refer patients to other services/agencies as appropriate

• Work with medical, nursing and health care assistant colleagues to ensure that National

Service Frameworks are being delivered

• Work as part of multidisciplinary team within the practice and NCN/Local Health Board to

promote integrated and seamless pathways of care

• Provide safe, evidence based, cost effective, individualised patient care

• Support and manage health needs of women presenting for family planning and cervical

cytology consultations after appropriate training

• Accurately perform specialist clinical skills e.g. administration of subcutaneous and

intramuscular drugs, wound care, catheterisation

• Implement and participate in vaccination and immunisation programmes for both adults

and children

• Advise and administer vaccinations for patients travelling abroad

• Contribute to practice achieving its QAIF targets to sustain high standards of patient care

and service delivery

• Understand and apply legal issues that support the identification of vulnerable and abused

adults and to be aware of local procedures and policies

• Use a structured framework to manage and review patient’s complaints and identify

learning from critical incidents

• Participate in and make use of audit information relating to clinic results, QAIF points and

patient questionnaires

COMMUNICATION AND TEAM WORKING

The post holder will:

• Utilise and demonstrate sensitive communication styles, ensuring patients are fully

informed and consent to treatment

• Communicate and support families dealing with bad news, terminal illness and distressing

situations

• Communicate effectively with patients and carers, recognising the need for alternative

methods of communication to overcome barriers such as different levels of

understanding, cultural background and preferred ways of communicating.

• Utilise sources of support and guidance e.g. Patient Information leaflets and provide

information in an acceptable form

• Record accurate and complete records of patient consultation, consistent with legislation,

policy and procedures • Use accurate read codes in order to ensure easy and accurate

information retrieval for monitoring and audit process

• Act as an advocate when representing patients and colleagues

• Participate in practice team meetings • Maintain effective communication within the

practice environment and with external stakeholders

TEACHING MENTORING AND DEVELOPMENT ROLE

The post holder will:

• Promote a learning environment for patients, nurses and other health professionals
