Job summary
The long-established Rycote Partnership of Thame are excited to be taking on Chiltern and Brook surgeries in Watlington & Chalgrove, creating The Rycote Group. We are seeking to appoint new practice nurses to bolster our team, joining 7 that already work across the three sites. You will join an expanded MDT team alongside GPs, NUrse Practitioners, HCAs, paramedics, pharmacists and social prescribers. This is an excellent opportunity for an experienced practice nurse to help shape the structure and culture of the new organisation or for a dedicated nurse looking for a new challenge.

Main duties of the job
The successful applicant will ideally be an experienced practice nurse with training in the management of chronic disease(s). Additional training will be available for the right candidate.
The successful applicant will:
Plan and provide appropriate nursing care in conjunction with other nurses, medical staff and the wider MDT team Carry out investigations, assess and deliver evidence based practice in accordance with practice policy and protocols Offer advice and health education to ensure disease treatment is complimented by health promotion Provide wound management including both chronic and postoperative wounds
Record Doppler studies, measurement and fitting of compression hosiery Administer adult and childhood immunisations, and other injections under Patient Group Directions Provide contraceptive advice and cervical screening
Provide travel health advice and immunisations
Undertake diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy
Participate in the delivery of various chronic diseases where appropriately trained including asthma, COPD, hypertension and diabetes
Ensure that current evidence based approaches to patient care are standard practice and participate in the development of practice protocols to advance nursing roles, enhance safety and improve patient care
Provide mentorship and support to Nurse Associates and HCAs

Nestled in at the foot of the Chiltern Hills, there is beautiful countryside on the doorstep, and excellent road links via the M40. Watlington is a historic market town, renowned for its thriving independent high street, and is a great place to live and work. Chalgrove is 10 minutes away and is also surrounded by picturesque scenery.

Job Summary:

Clinical Responsibilities
To assess, plan, implement and evaluate the needs of patients to include treatment, preventative care, appropriate screening and education. Undertake patient consultations, carry out physcial examinations, investigatory procedures and collection of specimens Manage annual reviews of patients with long-term conditions (where appropriately trained) and undertake interim support to ensure that the condition is well controlled Provide wound management including both chronic and postoperative wounds
Administer adult and childhood immunisations, and other injections under Patient Group Directions ensuring safe storage, rotation and disposal of vaccines and Liaise with other health professionals both within the practice and in other organisations to ensure integrated care Ensure that current evidence based approaches to patient care are standard practice Undertake diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy Provide contraceptive advice and cervical screening To provide health promotion advice To provide travel health advice and immunisations Undertake annual and interim chronic disease reviews in accordance with patient need and practice recall procedure Provide first aid and management of emergencies Undertake annual training in anaphylaxis and resuscitation techniques Ordering childhood immunisations, ensuring adequate supplies and stock rotation, completing spreadsheets and monitoring fridge temperatures Provide dementia screening Record Doppler studies, measurement and fitting of compression hosiery
Person Specification
Experience Essential
Experience of working as a practice nurse, ideally in currently in a primary care setting. The ability to work safely according to protocols and have sound clinical reasoning and decision making.

Qualifications Essential
Current registration with the NMC (including up to date revalidation)
