Job summary
We have an exciting opportunity for a qualified experienced GP Practice nurse to join our Primary Care Network team.
PCN Practice Nurse equipped with the knowledge, understanding, skills, attitudes, and behaviours to support general practice. The PCN Nurse will assist with the delivery of health promotion, phlebotomy, chronic disease management, immunisation and vaccinations and other clinical roles in line with their training and level of experience. The PCN Nurse will have a key role in supporting delivery of the QOF and Network Contract DES Service specifications
Main duties of the job
Deliver evidence-based nursing care within primary care settings.Undertake chronic disease reviews (e.g. Diabetes, Hypertension, Asthma and COPD).Provide immunisations, vaccinations and cervical screening.Support health promotion and preventative care initiatives.Participate in quality improvement, clinical audits and service development.Maintain accurate clinical records and uphold professional standards.

INTRODUCTION TO THE PCN
The Medics Primary Care Network (PCN) serves a population of under 64 525 patients residing across the Luton in Bedfordshire.
Working collaboratively with other partners to strengthen and support the delivery of Primary Care services, the PCN consists of 5 core GP practices Woodland Avenue Practice, Medici Medical Practice, Gardenia & Marsh Farm Practice, Bell House Medical Centre & Barton Hills Medical Group.
Medics PCN have a dedicated network site where many of the MDT, personalised care team and clinicians work collaboratively to deliver a pro-active approach to patient care.

Key Responsibilities and Duties:
Work within the PCN to provide and monitor General Practice Nursing care and Long term conditions monitoring alongside the wider PCN team. contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision.
Improve safety and quality of care at every opportunity.
Undertake additional clinical training as required by the PCN to support QOF and the PCN Network Contract/DES Services Specification
Support all clinical and non-clinical staff and the wider team
To support with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the PCN or to secondary/specialist care as appropriate. To implement and evaluate individual treatment plans dependant on training and experience.
Provide support to the PCN and Practices with regards to their QOF and Network Contract DES Services Specification including CVD prevention and tackling health inequalities
Develop relationships across the PCN to support integration of the role across health and social care: including community care, secondary care, and mental health services.
Provide support and supervision to trainee nursing associates, healthcare assistants, apprentices, and those on learning placements appropriate to level of competence.
Accurately perform and record clinical observations such as blood pressure, temperature, respiration, and pulse rates.
Engage positively with patients/carers/families and staff members promoting mutually respectful and appropriate relationships.
Communicate effectively with patients regarding providing and planning their care and carrying out risk assessment needs as required.
Communicate complex and sensitive information verbally and in writing.
Adapt and tailor communication skills to the need of the listener.
To act as patient advocate during practice or PCN meetings, sharing any relevant information confidently and accurately, maintaining patient confidentiality appropriately
Develop an understanding of MDT structures and members roles.
Support patients to improve and maintain their physical, mental, and behavioural health and well-being.
Provide advice to patients including promoting self -management where appropriate and signposting patients to other PCN colleagues, local community, and voluntary sector services.
Maintain accurate and contemporaneous patient health records.
Skills and training need to be updated through continuous professional development, attending regular educational updates, via e-learning and face to face sessions as appropriate
Wound management, undertake dressings and suture and clip removal.
Manage long term conditions such as diabetes, asthma, COPD and hypertension.
To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
As required to provide routine nursing care to patients in accordance with clinical based evidence, NICE Guidelines.
To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
To ensure the appropriate maintenance and use of chronic disease registers.
To ensure compliance with PCN policies and procedures.
To support the team in dealing with clinical emergencies.
To implement vaccination programmes for adults and children.
To support the clinical team with all safeguarding matters, in accordance with local and national policies.
To provide support to Practices in the PCN with management of chronic diseases such as Diabetes, COPD, Asthma, Hypertension, by providing clinical sessions.

Person Specification
Experience Essential
Experience in health and or social care. Communication and relationship building. Experience of developing relationships with a wide variety of people. Highly organised. Experience of working autonomously Experience of infection prevention and control measures. Experience of managing elderly and housebound patients.
Desirable
Experience of health and social care assessments. Demonstrated successful outcomes delivery within determined timeframes.
Qualifications Essential
Essential: Registered Nurse with the NMC With General Practice Experience and Chronic Disease checks experience. Knowledge of GP QOF. Demonstrable commitment to professional and personal development.
Desirable
Basic/advanced certificate of life support. Undertaken mentorship training. Awareness of the Transforming Nursing plan
