Community Health Specialist at Cxb. Location: New York City or Staten Island or The Rockaways or New York or Connecticut. Role: carrying caseload, conducting outreach, coordinating care Requirements: Master's degree (MSW, MHC, MPH, or MPA) required; LMSW or LMHC preferred. Experience with Housing First, Harm Reduction, CTI, Motivational Interviewing, crisis intervention, caseload management, documentation, and stakeholder coordination. Valid NY/NJ/CT/PA driver's license and Microsoft Office proficiency required. Bilingual preferred.
