About this role
Position Summary The Outreach and Intake Case Manager will determine Outreach Services program eligibility and will provide an assessment of social, psychological, and environmental factors in order to determine the needs of the client for linkage to needed services/resources. Working with the client, client's significant other, family, faith based community and other community providers, the case manager will provide/facilitate access to resources to meet the client's needs. Client follow-up is an integral function of job.
