About this role
The oncology nurse navigator (ONN) is a professional registered nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, the ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout phases of the cancer continuum. The ONN provides comprehensive care including care coordination and self-management building skills to promote better outcomes for patients. Responsible for compilation of pertinent medical records, comprehensive assessments, care planning, clinical interventions/procedures, patient satisfaction and desired outcomes achieved in treatment plan.
