About this role
Working closely with school districts and the Sonoma County Juvenile Probation Department, the Case Manager offers targeted case management, skill-building, and connects families to essential resources. As a Case Manager, your role involves advocating for services across community, social, educational, and health systems to reduce absenteeism and enhance family and school involvement. Additionally, you'll contribute to school interventions, district-wide events, and innovative attendance solutions. This position requires onsite presence at partner districts and throughout Sonoma County to support K-12 clients with dedication and expertise.
