Sea Mar’s HIV Services Specialist serves people living with HIV who experience high levels of systemic barriers to sustained engagement in health care, to ensure that persons living with HIV have access to assistance that enables them to gain or maintain outpatient/ambulatory health services, support services, and HIV treatment. Due to the systematic challenges faced by communities highly impacted by HIV, this position will work to support the needs of individuals who are newly diagnosed with HIV, individuals previously diagnosed with HIV, individuals that are marginally in care, and individuals that are out of care, to help reduce the barriers clients experience in accessing HIV care and other supportive services.

ESSENTIAL DUTIES AND RESPONSIBILITIES: • Knowledge and understanding of HIV case management program guidelines and standards, stays up to date on case management content, policies, and procedures. • Complete comprehensive assessment with Client and assist with screening and intakes. • Create, monitor, review, and update individual service plans every 6 months that are culturally responsive and timely to the needs of the Client. • Cultivate and maintain a supportive effective working relationship with Client. • Support clients through applied knowledge of healthcare benefits, public and private financial assistance, and insurance access • Attend HIV case management meetings/trainings, grant related trainings and other relevant trainings to maintain a comprehensive understanding of HIV and the communities being served. • Collaborate with colleagues and partners to improve health literacy for clients and improve access to care. • Develop and maintain a thorough knowledge of eligibility and benefit provisions for public and private financial assistance and health care coverage programs and provide assistance to support Clients to access these services. • Accurately, complete, and current records within Progress Notes in Provide database per HIV Medical Case Management standards and requirements. • Maintain comprehensive understanding of HIV Medical Case Management to include standards and the requirements.

Qualifications • Bilingual in Spanish and English preferred (verbal and written), but not required. • Must be proficient in MS Word, Excel, and PowerPoint. • Knowledgeable about and sensitive to the needs persons diagnosed with HIV. • Good organizational skills • Well-developed communication skills and team player • Flexibility with schedule: although typical schedule is Monday-Friday 8am-5pm, some evenings and/or weekend work will be required. • Must be a self-starter and team player. LANGUAGE SKILLS • Must have the ability to read, speak, and translate English and Spanish languages. REASONING ABILITY • Must be able to problem solve for the clients. • Must also have the reasoning ability to refer clients to appropriate agencies for community services after an assessment of needs is taken. Education, Certificates, Licenses, Registrations, and Medical Screening EDUCATION and EXPERIENCE • A Bachelor’s degree in public health, health care administration, public, or business administration, psychology, social work, or allied field and three years of professional management or consultative experience in a health services program that includes: o One (1) year providing case management or other direct client-level services. o One (1) or more years (full-time equivalency) of professional level experience in a health services program AND o One (1) or more years (full-time equivalency) of professional level experience in one or more of the following HIV, Hep C, STDs, and Opioid Overdose AND/OR o One (1) or more years (full-time equivalency) of professional level experience providing technical assistance in-person and by phone and computer AND/OR o One (1) or more years (full-time equivalency) working within a culturally diverse environment. OR o Seven years of professional management, consultative experience or lived experience in a health services program that includes. o One (1) year providing case management or other direct client-level services. o One (1) or more years (full-time equivalency) of professional level experience in a health services program AND o One (1) or more years (full-time equivalency) of professional level experience in one or more of the following HIV, Hep C, STDs, and Opioid Overdose AND/OR o One (1) or more years (full-time equivalency) of professional level experience providing technical assistance in-person and by phone and computer AND/OR o One (1) or more years (full-time equivalency) working within a culturally diverse environment Hourly - Hourly Plan, 30.00 USD Hourly
