For more detailed information, please visit our benefits page at <a style="color:#0563c1;text-decoration:underline" href="https://careers.sedgwickcounty.org/content/Health-and-Wellness/?locale=en_US">SCBenefits</a>.</span></span></span></p><p> </p> <p>Provide goal-directed, medically necessary peer support services to adults with severe and persistent mental illness to assist them to remain in the least restrictive environment.</p> <p> </p> <p>Service Provision Case Management Services</p> <ul> <li>Peer Support using identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being</li> <li>Help consumers regain the ability to make independent choices and to take a proactive role in treatment</li> <li>Share “lived experience” and model successful behavior and strategies</li> <li>Provide transportation to support client attendance in medical appointments and other therapeutic goal related activities, including development of community resources, employment or education access, social or consumer-run activities, and related supports.</li> </ul> <p> </p> <p>Documentation</p> <ul> <li>Complete progress notes in accordance with COMCARE and CCBHC guidelines which meet requirements for medical necessity, goal-directed treatment and client response to treatment within prescribed timelines</li> <li>Complete Notes for Record, Precautions statements and authorizations for release of records as required and/or recommended</li> <li>Enters no show and cancellations for accuracy and completeness of medical record</li> </ul> <p> </p> <p>General Administrative Duties</p> <ul> <li>Participate in multidisciplinary or peer team meetings to improve treatment and develop intervention strategies as needed</li> <li>Enter AIMS data as needed</li> <li>Attend staff meetings and individual/group supervision</li> <li>Travel to and from sites in personal transportation</li> <li>Identify and attend continuing education/training to enhance or expand practice and implement best practices related to the employee s area of practice within COMCARE</li> </ul> <p> </p> <p>Minimum Qualifications: High School diploma or equivalent. Must self-identify as a person in recovery from a behavioral health condition. Must be able to achieve and maintain designation as a Certified Peer Support Specialist for the State of Kansas, including completion of all required training within 6 months of hire. Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, and adult abuse registry. Must complete orientations provided by Sedgwick County and COMCARE. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must have access to personal passenger vehicle to complete the driving duties of the position.</p> <p> </p><p>Applicants have rights under Federal Employment Laws.
