The CCBHC Navigator assists individuals transitioning between different treatment programs and especially between different levels of care, provides mutual support and self-help groups, peer support for special populations, peer education and leadership development, and peer recovery services. The CCBHC Navigator may additionally support in immediate Social Determinants of Health needs, such as financial or housing needs.
