Works closely with other community agencies and service providers to maximize health outcomes for individuals, families, and communities. The majority of services provided are home-based.</span></p></div></div><div style="padding:10.0px 0.0px;border:1.0px solid transparent"><div style="font-size:16.0px;word-wrap:break-word"><H2 style="font-size:1.0em;margin:0.0px"><b>Essential Duties</b></H2> </div><div><ul> <li style="font-family:arial, helvetica, sans-serif"><span style="font-family:arial, helvetica, sans-serif">Provides therapeutic social work intervention to diverse client system which may include but not limited to crisis intervention, community resource referrals, client advocacy, support, outreach, follow-up, education, and intra/inter agency consultation. </span></li> <li style="font-family:arial, helvetica, sans-serif"><span style="font-family:arial, helvetica, sans-serif">Participates in a multidisciplinary team effort to coordinate/implement services that will ensure appropriate treatment plan continuity of care for all clients/patients served of care. </span></li> <li style="font-family:arial, helvetica, sans-serif"><span style="font-family:arial, helvetica, sans-serif">Serves as social work services resource/consultant person concerning appropriate management of social situations that affect the effective functioning of individuals and families. </span></li> <li style="font-family:arial, helvetica, sans-serif"><span style="font-family:arial, helvetica, sans-serif">Prepares written reports and clinical records (progress notes, summaries, statistical forms, etc.) of services rendered; responds timely to requests for information. </span></li> <li style="font-family:arial, helvetica, sans-serif"><span style="font-family:arial, helvetica, sans-serif">Assumes advocacy/liaison/collaborative role for patients/clients via contacts with community agencies/personnel, hospital personnel, court disposition hearings, abuse cases, etc.; collaborates with inter/intra agency personnel involving referral activities. </span></li> <li style="font-family:arial, helvetica, sans-serif"><span style="font-family:arial, helvetica, sans-serif">Maintains a current file of referral sources;
