Shift: Shift 1 FTE: 1.000000 Summary: Provides screenings and assessments to women and their families eligible for the healthy start program within patient homes. Assesses individual patient needs for resources to create care plans and arrange for coordination referrals for services that eliminate social detriments of health.
Education: Essential: * Bachelor’s degree in Social Sciences Psychology Social Work or Healthcare Administration. Education specialization: Essential: * Social Science * Science in Psychology * Social Work * Health Care Administration
Experience: Essential: * One Year of related experience
Credentials: Essential: * Basic Life Support within 30 days of hire
