How you’ll make an impact in this role

• Develop and Implement Care Plans: Assess patient needs, interpret Continuous Glucose Monitor (CGM) and lab data, and design individualized diabetes management plans in collaboration with the care team. • Provide Patient and Family Education: Instruct patients on lifestyle modifications, nutrition, insulin administration, and oral medications to promote successful self-management and prevent long-term complications. • Optimize Clinical Outcomes: Actively monitor glycemic trends, track hemoglobin A1c levels, and adjust treatment protocols or titrate medications under physician supervision to achieve target blood sugar ranges. • Coordinate Interdisciplinary Care: Collaborate with endocrinologists, primary care providers, dietitians, and pharmacists to ensure seamless, holistic support and reduce the need for hospital readmissions. What minimum requirements you’ll need Licensure / Certification / Registration:

• Registered Nurse credentialed from the Oklahoma Board of Nursing required. • Advanced Life Support preferred. American Heart Association or American Red Cross accepted. • BLS Provider preferred. American Heart Association or American Red Cross accepted. • Certified Educator specializing in Diabetes Management preferred. Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire. What additional preferences we're seeking
