Job summary

We are inviting expressions of interest for an exciting opportunity that has arisen for an experienced and highly motivated clinician to take on the role of Clinical Coordinator within our Crisis Resolution and Home Treatment Team.

The post holder will play a key role in supporting safe, effective, and timely decision-making, ensuring that individuals receive the right care, at the right time, in the least restrictive setting possible.

We are seeking a confident, adaptable practitioner with excellent clinical assessment skills, sound judgement, and the ability to support teams managing complex and high-risk presentations. The successful candidate will work alongside the multidisciplinary team to coordinate service activity, manage demand and capacity, and ensure robust gatekeeping and admission avoidance processes are embedded across the pathway.

Main duties of the job

The Crisis Care Centre is an essential component of the urgent care pathway and provides an all age single point of access to mental health services in norths Staffordshire, whilst also offering a 24-hour, seven-day crisis service with the additional offer of intensive home treatment as an alternative to an acute inpatient admission. The Crisis Care Centre in line with the NHS Long Term Plan will be expanding this offer too support individuals in crisis by receiving referrals via NHS 111, in addition to the dedicated line for ambulance, police, GP's and other professionals requiring an urgent mental health response.

The post holder will be expected to provide strong clinical leadership as well as work as a member of the multi-disciplinary team within the Crisis Care Centre co-ordinating a fast-paced dynamic service on a shift-to-shift basis over a 7-day 24-hour period.

The post holder will ensure that all referrals received into the service are triaged and assessed according to risk and needs, ensuring that appropriate advice and intervention is offered.

The post holder will require relevant links and understanding of wider mental health and discharge pathways.

Regular audits are carried out that inform staff of their performance against clinical standards.Service improvement activities are in place.Research activity is actively encouraged, and all staff are able to critically examine their practice against best evidence.All staff are engaged in quality improvement activities and develop the skills to be able to deliver quality improvement projects. At least one quality improvement project is underway at any point in time and that there is service user and carer involvement with this.

The post holder will:

Demonstrate skills and understanding of the supervision process ensuring the service has a culture that embraces supervision.Participate in reaching and learning activities within the service.Promote professional practice in accordance with relevant Codes of Professional Conduct and maintains their requirements for regular re-registration and revalidation.Identify areas for personal development to improve and enhance knowledge and skills.Develop leadership skills, contributing to decision making about practice using training and development opportunities provided by the Trust.Contribute to decisions about care through membership of professional networks at unit, professional network and/or organisational levels.Maintain awareness of current developments and the evidence base for practice in their clinical area. Acknowledge their own limitations and identifies their training and development needs with their Line Manager.Make effective use of supervision with their line manager on a regular and agreed basis, using reflective practice and recognising emotional and mental effort required to fulfil the post.Manage own time efficiently.Be aware of, and adhere to, all Trust Policies, acting as a role model to other staff.Exercise a high level of professional judgement and support the development of the team.

Resource Management-The post holder is responsible for: Ensuring that systems are in place to manage unit resources, both human and financial. Ensuring that budgets are approved and that they have a working knowledge of this alongside the Operational Team Manager.

Person Specification

Qualifications Essential

A registered mental health practitioner with a degree in nursing, social work or occupational therapy or equivalent experience Evidence of continuing professional development. Management/Leadership Qualification or equivalent experience of management/leadership

Experience Essential

Significant experience of: Post qualification work in core profession. Experience of manging clinical practice at band 6. Experience of leading on Safeguarding issues including providing advice and guidance to other staff. Experience in supporting clients with complex needs. Experience of clinical practice development. Experience of implementing and monitoring standards. Experience of managing clinical risk effectively. Experience of leading and managing a multidisciplinary team. Experience of providing clinical and managerial supervision. Experience of working in a multicultural environment and knowledge of methods to develop culturally sensitive practice. Experience of recruitment and selection of staff. Evidence of undertaking audit work.

Knowledge Essential

Specialist clinical knowledge of medical and psychiatric conditions, diagnosis, treatment and care including contraindications and side effects to medications, relevant to health speciality. Knowledge and understanding of Recovery Model of Care and any other speciality relevant models. Up to date knowledge of relevant legislation and quality framework associated with the clinical speciality (e.g. MHA, clinical governance). Understanding and appreciation of safeguarding procedures. Understanding of the role and function of other organisations providing services to people with mental health needs and how these complement services the NHS provides. Knowledge of performance management procedures. Awareness of planning & developing services processes e.g. contributing to pathway development. Standard keyboard skills and working knowledge of MS Office suite, including Outlook, Word and Excel
