Job summary
We are seeking a Registered Pharmacy Technician to join our Medicines Optimisation team!
The Medicines Optimisation team support people in taking their medicines safely and effectively. The aim of the service is to improve individual outcomes by optimising medicines use. The Pharmacists and Pharmacy Technicians are also available to healthcare professionals for advice on specific individual medication issues, particularly for complex individuals.
You will work with the team to support people at home. You will play a key role in supporting the safe transfer of care between care settings, preventing readmission for people recently discharged from hospital, supporting people to regain independence with medication and recommending short or longer term options to aid compliance.
You will hold a dual-locality role that adapts alongside our organisation. Initially, your week will be split between two key areas: three days a week in inner-city East Bristol, and two days a week supporting our established services in South Gloucestershire. While the East Bristol base is a permanent fixture of the role, the remaining two days are designed to be flexible, allowing you to bring your expertise to different teams or locations as theteam continues to develop its service
Main duties of the job
The pharmacy technician will:
Carry out medicines reconciliation for patients following discharge from hospital and rehabilitation units Attend and actively participate in MDT meetings within the Integrated Neighbourhood Teams (INT teams) Provide advice and guidance to clinicians Visit patients at home to carry out medication compliance assessments and put appropriate solutions in place to maintain independence. Escalate clinical concerns to the most appropriate clinician. Attend service and organisation-wide meetings to raise the profile of medicines optimisation. Work collaboratively with system colleagues to improve patient safety within the community pathways

Qualifications Essential
Registered as a pharmacy technician with the GPHC Accredited Medicines Optimisation Technician (PWDS or equivalent) BTEC or NVQ 3 in pharmaceutical services or equivalent
Desirable
Have completed the Enhanced clinical practice for pharmacy technicians course (previously Clinical prioritisation Course ) Service
Skills, Knowledge & Experience Essential
Extensive post qualification experience as a pharmacy technician Previous medicines optimisation experience including assessment of patients own drugs and medicines reconciliation Experience of working with partner organisations Up to date knowledge of medicines optimisation and the safe and secure handling of medicines Good clinical knowledge Excellent communication skills, both written and verbal Excellent problem identification and solving skills. Knowledge and understanding of information sources and retrieval
Desirable
Experience in Community Health Services or Primary Care
