Job summary
General Practice Alliance (GPA) are looking for a proactive and people-focused Pharmacy Care Coordinator to join our friendly team, working within Grand Union PCN.
This role is perfect for someone who enjoys supporting patients, working closely with clinical teams, and helping to improve how people understand and manage their medicines.
This is a full-time role, though part-time hours will be considered.

Main duties of the job
As a Pharmacy Care Coordinator, you'll be the key link between patients, practices, and our pharmacy team. You'll help identify individuals who may benefit from additional support, prepare patients for clinical reviews, and ensure they can access the right services at the right time.
You'll also play an important role in coordinating care, supporting personalised care plans, and helping maintain safe, effective medicines management across the PCN.

Job description Job responsibilities
Enable access to personalised care and support:
Work with the pharmacy team to proactively identify people who could benefit from their support, utilising the resources available to them. E.g. Ardens searches, Eclipse, building new searches.
Increasing patients understanding of how to manage and improve health and minimise harm from their medication.
Develop an in-depth knowledge of the local health and care infrastructure, with expertise in the pharmacy team and know how and when to enable people to access support and services that are right for them.
Support people to develop and implement personalised care and support plans, to prepare them and support them in making the most of interventions in the rest of the pharmacy team. E.g. ensuring drug monitoring and basic observations are taken prior to structured medication review.
Review and update personalised care and support plans at regular intervals and ensure they are communicated to other professional involved in their care.
For care home patients, ensuring patients have had admissions review of medication on admission to the care home and a minimum of annual structured medication reviews. Setting up proxy-access and proving a direct link between the care home and the pharmacy team.
Uploaded to the relevant online care records, with activity recorded using the relevant SNOMED codes.
Co-ordinate and integrate care:
Regularly liaise with the pharmacy professionals in the PCN who are involved in the persons care, facilitating a co-ordinated approach, and ensuring everyone is kept up to date so that any issues or concerns can be appropriately addressed and supported.
Refer to pharmacists, pharmacy technicians, booking patients in a suitable slot for their need and preparing them to make the most of the consultation. E.g. providing information about their condition or medication, ensuring blood tests and basic observations are up to date.
Refer to the Social Prescribing Link Workers, Health and Wellbeing Coaches, statutory or community services where appropriate, based on what matters to the person.
Refer to clinical colleagues outside the pharmacy team where there is an unaddressed clinical need which the pharmacy team is unable to support with, helping to ensure patients receive a joined-up service and the most appropriate support.
Actively support and uphold excellent medicines management within the PCN
Actively participate in multidisciplinary team meetings in the PCN including safeguarding meetings.
Fulfil clinic coordinator role for group consultations, where clinical pharmacy team are clinicians.
Person Specification
Skills and Knowledge Essential
Knowledge of how the NHS works, including primary care and PCNs. Strong organisational skills, including planning, prioritising, time management and record keeping.
Desirable
Basic knowledge of long -term conditions and the complexities involved: medical, physical, emotional, and social.
Experience Essential
Experience of working within multi-professional team environments. Experience of working in health, social care or other support roles in direct contact with people, families or carers (in a paid or voluntary capacity). Ability to listen, empathise with people and provide person-centred support in a non-judgemental way.
Desirable
Experience or training in working with medication from practice or pharmacy work. Experience of data collection and using tools to measure the impact of services.
Qualifications Essential
Educated to GCSE Level (5 GCSEs at grade C/4 or equivalent inc. English Language and Maths).
Desirable
Qualification in Health and Social Care.
Job Circumstances Essential
Reliable transportation to facilitate a hybrid work arrangement and occasional travel for mandatory meetings as required by the position. Possess the legal right to work in the UK and ability to provide appropriate documentation for verification.
