This posts is a cross county post covering Coventry and North and South Warwickshire, where the main hub will be at Stratford upon Avon

The service is a 7 day a week service between the hours of 08.00-20.00

The core working hours will be Mon-Fri 09.00-17.00, some flexibility will be needed at times to cover the 7 day a week working pattern, including long days based on the service need.

The Team Manager for the Hospital at Home Service plays a pivotal role in leading a multidisciplinary team to deliver safe, effective, and compassionate care to patients within their own homes. This role ensures that patients who would otherwise require hospital admission receive care within their own home whilst supporting a strengths-based approach to recovery, improving patient outcomes.

The post holder will work closely with senior clinicians, acute hospital partners, and external agencies to maintain seamless patient pathways and uphold the highest standards of care.

Good communication and problem-solving skills are essential, as well as knowledge of the needs of older adults. We also are looking for people who can demonstrate the Trust's vision and values in practice and are motivated to be involved in innovation and service development.

Main duties of the job

Provide day-to-day leadership for the Hospital at Home team, ensuring safe, effective delivery of care in patients' homes.

Coordinate daily staffing rotas, caseload allocation, and referral flow to maintain service responsiveness and continuity.

Lead multidisciplinary handovers, and escalation processes to ensure timely clinical decision-making.

Oversee incident reporting, investigation, and learning, ensuring a culture of openness and continuous improvement.

Monitor clinical risk, safeguarding concerns, and patient safety issues, escalating appropriately.

Provide visible, supportive leadership to clinical and administrative staff, promoting a positive team culture.

Deliver regular supervision, appraisal, and competency oversight for team members.

Identify training needs and support professional development, ensuring staff maintain required skills for community care.

Build strong working relationships with acute hospitals, community services, GPs, social care, ambulance services, and voluntary sector partners.

Ensure seamless patient pathways, including admission avoidance and early supported discharge

Communicate effectively with patients, families, and carers, ensuring they understand care plans and feel supported.

Job description Job responsibilities

As the successful candidate you must have a professional qualification i.e., nursing, occupational therapist or social worker, with substantial post registration experience as a qualified professional of which you will have recent experience working with clients within a secondary care mental health setting.

Communicate with both internal and external stakeholders, such as service users, carers, other teams, third sector and voluntary organisations on matters which may be complex and/or contentious.

Communicate effectively across a wide variety of settings, using tact and diplomacy in response to situations that may be complex and sensitive and may have resulted from conflict.

Providing advice and use their initiative and follow organisational procedures in routine and crisis situations

Apply communication skills which take account of other communication needs and differences, in order to plan, deliver and evaluate care.

Apply enhanced communication skills both oral and written to communicate information within records and reports within their service area. This may also include the need to respond to and offer and reassurance to questions and concerns from a variety of groups, such as service users, carers and other staff/teams.

Comply with and implement local and Trust policies and procedures regarding appropriate communication.

Be required to lead client reviews within the multi-disciplinary team and act as an advocate for service users

Analyse and act on multiple and complex information assuring adherence to local and national guidance, legislation and policy

Act within the limits of their competency and authority in line with their professional code of conduct.

Take responsibility and actively participate in both managerial and professional supervision to ensure their competence and support their continuous professional development.

Provide management and/or clinical supervision to staff within their team.

Deputise in the absence of a senior manager as required.

Manage change in a complex environment responding to varying needs.

Ensure that all organisational systems and processes are in place, adhered to and monitored within area of responsibility.

Will manage and organise competing and conflicting priorities.

Monitor the effectiveness of others time management in order to ensure safe effective delivery of services.

Plan and manage a range of activities which may be complex in nature and delegate appropriate responsibilities to other members of the team.

Review, evaluate and adjust activities in response to changing demands

Positively manage the varying demands of the team within which they work, including complex situations, complaints and adverse incidents.

Have the ability to use I.T. systems sensibly and purposefully to meet the needs of the role and will be required to utilise systems safely and securely in line with organisational guidelines.

Attend annual training and undertake life support training appropriate to role

Person Specification

QUALIFICATIONS Essential

Relevant Professional Qualification i.e RMN, DipCOT Current unrestricted registration with HCPC/NMC Specialist expertise across a range of procedures Professional knowledge acquired through degree/diploma Specialist managerial training Evidence of continuing professional development

KNOWLEDGE & SKILLS Essential

Excellent written communication skills

EXPERIENCE Essential

Substantial experience in a Community Mental health care setting Significant experience of risk assessment and positive risk management

OTHER Essential

Must have full driving licence and access to a motor vehicle for business use
