The post holder will be a key member of our multidisciplinary teams including a Consultant Geriatrician, Advanced Nurse Practitioners, Registered General Nurses, Paramedic Practitioner, Healthcare Assistantsto provide an urgent community response serviceto patients who are experiencing a health crisis with the aim to supportand provide virtual ongoing management tothem in their own homes (including care homes) rather than being admitted to hospital.

You will be a team player and work collaboratively with staff from all disciplines whilstmaintaininghigh professional standards. You will ensuregood communicationand effective processes and procedures aremaintainedacross the service.

The post holder will work37.5hrs per week, 7 days a week, between the hours of 8am - 8pm, which will include weekends.

The role isbased atWokingham Hospital,Wokinghamand staff parking is available on site

Main duties of the job

The role will comprehensively assess patients with complex physical, mental, and social care needs using advanced clinical examination skills, detailed history taking and biometric monitoring and evaluation skills.

To undertake new patient assessments and reassessments in a timely manner, reporting back to the senior staff members for further discussion and/or assessment.

To analyse signs and symptoms, laboratory tests and other measures of function, in formulating a diagnosis

Develop individualised treatment and management plans using evidence-based practice and proactively monitor patients and review care plans

To prescribe and install equipment in line with Millbrook & Community Equipment Stores competencies framework and to demonstrate to the patients the correct use of equipment, if required

Maintain accurate, contemporaneous, and comprehensive records and ensure that reporting arrangements are adhered to.

To develop and participate in planned teaching programmes of training for students and new members of staff.

To be able to effectively manage crisis situations including those that feature violence or aggression with special regard to lone working in the community and out of hours

To promote collaborative working with health and social care colleagues, voluntary agencies, and other relevant organisations.

Registered General Nurse or Paramedic

Advanced History taking and physical assessment, or to be successfully completed within a 1-year timeframe of undertaking post

Able to work autonomously, with the ability to work under pressure, with excellent organisational, communication and time management skills to prioritise work

Person Specification

Education/Qualifications/Training Essential

Relevant clinical qualification with current registration (e.g. RGN, Paramedic, Physician Associate) Practice Assessor Training - or to be completed 1-year post registration or staff new to community will do the training within 1 year (in post) History Taking & Physical Assessment Module L6/L7. To be successfully completed within a 1 year timeframe of undertaking post

Continuous Professional Development Essential

Evidence of on-going professional development Willingness to undertake further training and development (Practice Assessor Training and History Taking and Physical Assessment)

Previous Experience Essential

Post Registration experience in Community or acute setting

Desirable

At least 1 years post qualifying experience

Well developed written and verbal communication skills Able to demonstrate use of own initiative and ability to work alone in ever changing circumstances. Evidence of effective team working Teaching relatives and carers across all areas of care Computer literate Ability to demonstrate organisational skills Ability to demonstrate and show evidence of effective clinical skills Ability to manage distressed patients or relatives with terminal/palliative care needs. Ability to manage patients who have intravenous therapy

Additional Requirements Essential

Full UK Driving Licence and use of car for work purposes Ability to drive to multiple sites and work shifts covering 8am-8pm, 7 days a week including weekends and Bank Holidays. Manual dexterity required for the role
