Job summary
Following significant investment into the community hospitals wards we have an exciting opportunity to join our team. We are seeking a highly motivated and enthusiastic Band 3 Therapy Assistant to join our therapy rehabilitation team based in the North locality.
Applicants must be prepared to participate in a 6 day week service, covering bank holidays and Saturdays on a rota basis.
The role will have a main base of Shotley Bridge Hospital with the expectation of cross site cover as service demands into other hospitals within the North locality (Weardale Community Hospital and Chester-le-Street Community Hospital). This may include cross-site cover into the South locality hospitals (Sedgefield and Richardson) to support with staffing issues as needed. This can be further discussed at interview.

Main duties of the job
This is an exciting role with a great deal of patient contact and opportunities to increase and consolidate both skills and knowledge. You will work under direct supervision of the senior Occupational therapists and Physiotherapists. You will have the autonomy to carry out rehabilitation following qualified or band 4 assessment as well as assisting to facilitate timely discharges and maximise patients' independence. You will act as a source to provide health education and advice to patients, carers, and their families.
The post will combine competencies from physiotherapy and occupational therapy disciplines, to support the delivery of the therapy services to patients within a community rehabilaition ward setting

Person Specification
Qualifications Essential
Minimum of 4 GCSE level C, or equivalent, including English Language NVQ Level 3 in relevant area, e.g diagnostic and Therapeutic Care or Care or working towards or equivalent experience Knowledge of Microsoft packages
Desirable
An understanding of County Durham and Darlington NHS Foundation Trust, its culture, strategic direction, achievements and NHS standing
Experience Essential
Experience of working within multidisciplinary teams. Experience of working with adults. Recent and relevant assistant experience within rehabilitation in a related clinical specialism within the NHS.
Special Skills & Knowledge Essential
Knowledge of NMC / HCPC and accountability Ability to communicate clearly and effectively. Able to work as part of team. Able to observe and record physical and social changes in patients. Able to identify when to report to a registered therapist. Able to work alone with patients within an agreed care plan. Able to work with minimal supervision. An appreciation of dealing with people in their own homes. Able to maintain confidentiality. Understand of working with other agencies/multidisciplinary working.

Able to travel independently across Trust sites and the locality to meet the requirements of the post
