Main duties of the job

Working as a Pharmacy Technician at the Brooke Surgery will be a rewarding experience. The whole team values the role of technicians in primary care. You will be supported in developing your skills clinically by three GP partners, who are skilled trainers, and teach on pharmacist prescribing courses; and non-clinically, by an experienced management team. As a training practice, we place a great emphasis on teaching and ensuring that we pay forward our knowledge. We are looking for someone who is keen to develop themselves and contribute tot he team. Part of our team includes pharmacist, another technician and pre-registration pharmacists.

You will have a variety of duties, dealing with prescription queries, docman letter's from the hospital, audit work and support the pharmacy team.

Computer expertise is a must-have. You will be working with multiple IT systems (EMIS, DocMan, online system) that will be taught on induction.

Job Summary:

To support the Practice Based Clinical Pharmacist to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

The role will also include training and development of practice based prescribing co-ordinators and administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.

Undertake patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients. Carryout medicines optimisation tasks, supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

Support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies.

Support the Clinical Pharmacist in Structure Medication Reviews (SMR) i.e., organise necessary monitoring tests prior to SMR Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities. Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.

Implement efficient ordering and return processes and reducing medication wastage. Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS). Promotion of Electronic Repeat Dispensing (eRD) and online ordering.

Develop relationships with other pharmacy professionals and members of the multi-disciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health. Support practice reception teams instreaming general prescription requests, so as to allow GPs and clinical pharmacists to review the more clinically complex requests. Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.

Job Responsibilities

To undertake medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.

To support the achievement of the practices prescribing Quality and Outcomes Framework targets.

Working with the Clinical Pharmacist and Prescribing Cordinators to contribute to the review and development of repeat prescribing protocols /Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.

To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.

To participate in medication audits.

To assist in the appropriate monitoring and management of prescribing budgets and to liaise with the medicines management team at the Brooke

To prepare evidence-based resources and information to support the implementation of rational cost-effective prescribing.

To support the Practice Based Clinical Pharmacist to provide medication reviews, patient information leaflets, medicine awareness.

To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols.

To advise on the sourcing and safe management of medicines as appropriate.

Be responsible for the organisation, planning and of own workload to meet set deadlines.

To continue professional development to maintain the highest level of competency required for your role through annual updates and any training as required.

Always adhere to practice and professional guidance, policies and protocols in relation to your role and the care of patients.

To always have patient care as the highest priority, through using your expertise and skills to accurately assess patients individual needs and manage their treatment using practice protocols, and the appropriate guidelines.

Support other members the team at all times both clinical and non-clinical, including mentoring and teaching where desired.

Take responsibility for their own work, as well as their learning and development at all times.
