Job summary
The post holder will work within a specialist stroke inpatient rehabilitation team at Abingdon community hospital in Oxfordshire. Care is focused to empower clients to manage their conditions effectively and maximize their potential to reach their goals of self-management, access to their environment and return to previous occupations and involvement in the community.
The post holder will provide a high-quality rehabilitation service, in line with current best evidence-based practice, for stroke patients on the Oxfordshire stroke pathway.
Main duties of the job The clinician will complete assessments for stroke patients, including complex cases, using clinical reasoning to plan individualised rehabilitation. They will deliver and review treatment programmes in line with clinical findings, national guidelines, and best practice. They will independently manage and prioritise their caseload and maintain communication with senior physiotherapists for supervision and risk escalation. They will set patient-centred goals and discharge plans with patients, carers, and the MDT. Communication is adapted for patients with cognitive, sensory, or communication impairments. They will maintain accurate clinical records and ensure safe use and cleaning of equipment such as walking aids, electrotherapy devices, and gym equipment. The role requires managing clinical risk, including exposure to bodily fluids and challenging behaviours, while upholding dignity, safeguarding, and legislation such as the Mental Capacity Act. The clinician will maintain competence through CPD, mandatory training, and reflective practice, meeting HCPC requirements. They keep an updated portfolio, participate in annual appraisals, and take responsibility for ongoing development. They will act as a role model, supporting students, unregistered staff, and colleagues through coaching and guidance.
