Job summary
We are seeking a dedicated Clinical Pharmacist to lead medicines optimisation and provide expert clinical oversight across our network.
Please note the points below;
1. This is a FACE TO FACE role (with no option for remote or hybrid work).
2. This is a FULL TIME role (not part time or flexible hours).
3. EXPERIENCED Clinical Pharmacist who has completed CPPE Pathway.
4. You will be paid as a SELF EMPLOYED individual or as a Limited Company (not PAYE).
Main duties of the job Key Responsibilities
Complex Clinical Reviews:Conduct comprehensive, person-centered medication reviews for patients with chronic diseases and complex polypharmacy. You will be the clinical lead for our most vulnerable populations, including the elderly and those residing in care homes.
Specialist Liaison & High-Risk Monitoring:Take a central role in managing shared care protocols. You will collaborate closely with other healthcare professionals to monitor and reduce the inappropriate use of high-risk medicines, ensuring patient safety is paramount.
System Integration:Act as a bridge between general practice and the wider healthcare ecosystem.
Workload & Outcomes Management:Proactively manage GP workload by leading on medicines-related queries and self-care initiatives, focusing on reducing medicine waste and improving long-term patient outcomes.

Clinically assessing and treating patients face to face, using expert knowledge of medicines, for specific disease areas. Providing specialist expertise in the use of medicines and medicine information (i.e. side-effects, dosage) across the practice. Providing medication reviews, and health education advice, to patients at the designated practices by face-to-face, telephone or notes-based review, as agreed with the practice. Providing leadership on person-centred medicines optimisation and quality improvement. Processing specialist treatment letters. Processing hospital medication discharge letters. Acting as a key contact in the practices, for all medication related queries. Supporting improvements in clinical care, through practice-based audits and implementing change.
