Responsibilities:
Assessment: Evaluate patients for functional limitations and collaborate with patients, families, nurses, physicians, and other health team members to develop a personalized therapy plan.Direct Care: Provide therapy services to treat medical and surgical conditions using accepted physical therapy modalities.Education: Instruct and supervise patients, families, home health aides, and others on home exercise programs and aspects of patient care.Ongoing Evaluation: Continuously assess the patient's condition and adjust the therapy plan as needed.Communication: Report significant clinical information to physicians, clinical managers, and other team members. Document clinical evaluations, treatments, and patient responses to therapy.
Qualifications:
NY Professional License and current registration as a Physical Therapist.Minimum 1 year of full-time experience providing physical therapy services.Ability to assess and respond to patient and family needs in a home care setting.Home care experience is preferred but not required.Ability to use technology (such as tablets) for patient documentation and care.Experience with Oasis assessments is a plus.A valid driver's license and access to a reliable automobile for travel in the field.
Salary:
$95,000 to $108,000 annually, based on experience.
