This role offers an exciting opportunity to lead a multi-professional team providing support to people in their own homes within Restormel area. The Homefirst team provide a preventative, supportive and rehabilitative service, to avoid admission to an acute setting and to enable patients to recover at home safely following an acute period of illness or injury. The team also support early discharge from the acute setting, mitigating the risk of deconditioning within a hospital setting and to support patients to regain a level of independence and to remain at home safely. The team works closely with the neighbourhood integrated teams and other partner organisations, with the aim of boosting community capacity and aiming for a seamless transition between services for patients.

Main duties of the job
Key elements of the role are: day to day coordination and supervision of a team comprising of therapists, nurses, support workers and administrative staff; providing complex case management and support; monitoring and reporting on key performance indicators; identifying new opportunities for team and service development; ensuring that the service is cost effective and efficient; and pathway development with partner organisations.

Education/Qualifications Essential

Skills and Aptitude Essential
Previous involvement in implementing change in practice Evidence of leadership skills Clinical competencies in a range of skills ie advanced clinical assessment and care planning
Knowledge and Abilities Essential
Significant post registration experience
