Job summary <h5>Be the First Point of <span class="single_desc_int_gb_word">Support</span> That Makes a Difference</h5><p>Are you someone who thrives on helping others and making a real impact every day? As a Community Health Navigator, youll be at the heart of our service the friendly, knowledgeable voice that connects people to the right care at the right time.</p><p>Based in Chippenham or Swindon, youll join our BSW (Bath & North East Somerset, Swindon and Wiltshire) Community Health Services Single Point of Access (SPA) a fast-paced, supportive hub where no two days are the same. Youll be the first point of contact for patients, the public, healthcare professionals and colleagues, providing reassurance, guidance and solutions when they matter most.</p><p>Through phone, email and written communication, youll deliver a service that is not only efficient and professional, but also compassionate and person-centred. Youll help people navigate services, ensure referrals are handled smoothly, and play a vital role in keeping care flowing where its needed most.</p><p>This is a dynamic frontline role where your communication skills, attention to detail and ability to stay calm under pressure will truly shine.
