Job summary

The PCN Services Care Coordinator plays an important role within Maidenhead PCN, providing administrative, operational and coordination support to PCN teams and services. The role supports the delivery of projects, clinics, programmes and initiatives that improve the quality of care and enhance the health and wellbeing of local residents.

Working across St Marks Hospital and local GP practices, the postholder will work closely with GPs, practice teams, social prescribing link workers, health and wellbeing coaches, care coordinators, and other multidisciplinary team members within the PCN to ensure services are delivered effectively and efficiently.

Main duties of the job

The PCN Services Care Coordinator will provide essential administrative, operational and coordination support across Maidenhead PCN. The postholder will support PCN teams with scheduling, service administration, project delivery and day-to-day operational activities, helping to improve the quality of care and enhance the health and wellbeing of residents across Maidenhead.

Key Responsibilities

Provide comprehensive administrative and coordination support to PCN teams and services.

Support the scheduling, coordination and management of appointments, clinics, meetings, training sessions and patient-facing activities across PCN services.

Coordinate diaries and provide administrative support to clinicians and PCN staff.

Manage and maintain PCN clinical systems, including appointment books, clinic templates and patient records, ensuring services are scheduled and delivered effectively.

Manage referrals, service enquiries, correspondence and communications across PCN services, including the allocation and administration of referrals to appropriate multidisciplinary teams.

Support the management, cancellation, rebooking and invitation of patients to appointments and clinics, ensuring effective management of capacity and patient access.

Utilizing and managing Accurx messaging.

Support and manage the bookings of PCN services.

Monitor and manage patient communications and service requests received through digital and telephone systems, ensuring timely responses and resolution.

Support the delivery and administration of PCN clinical services, including coordinating appointments, managing patient pathways and supporting service activity.

Monitor missed appointments (DNAs), undertake follow-up actions and support the re-engagement and rebooking of patients where appropriate.

Maintain accurate, timely and confidential records in line with information governance and data protection requirements.

Support the organisation and delivery of PCN projects and programmes aimed at improving quality of care and resident wellbeing.

Work closely with GP practices and PCN teams to coordinate activities and ensure services run efficiently.

Assist with audits, service reviews and quality improvement activities.

Prepare meeting agendas, take minutes, maintain action logs and support follow-up actions.

Support community engagement activities, events and health promotion initiatives.

Support onboarding and coordination of staff and stakeholders involved in PCN initiatives.

Act as a central point of contact for administrative and coordination support across the PCN.

Monitor and manage clinical and non-clinical stock levels, consumables, and equipment at St Marks Hospital, working with PCN managers, clinicians, and other staff to ensure adequate supplies (including sharps bins) are available and maintained in accordance with relevant policies and procedures.

Work as part of the wider multidisciplinary team to support integrated and personalised care across the PCN.

Working with the wider PCN care coordinator team and providing appropriate support and cover (e.g. for annual leave)

Supporting PCN project work.

Monitoring lab reports and escalating any concerns in line with PCN policies/processes.

Person Specification

Qualifications Essential

Experience of working within healthcare, primary care, NHS, social care or other support services. Experience providing administrative, operational or project support. Experience of supporting people, families, carers or professionals in a customer-facing or support role. Strong organisational and written skills, including planning, prioritising, time management and record keeping. Experience managing competing priorities and workloads effectively. GCSE Grade AC (or equivalent) in English and Mathematics. Evidence of strong written and verbal communication skills.

Desirable

Experience within a Primary Care Network or General Practice setting. Knowledge of EMIS, Accurx or other clinical systems. Knowledge of Safeguarding Children and Vulnerable Adults policies and processes. NVQ Level 3 in Health, Adult Care, Business Administration or equivalent qualification, or willingness to work towards one. Experience supporting quality improvement or service development projects.
