Job summary
Pelham Medical Group is looking to recruit a Practice Nurse for 26 hours per week for 1 years maternity cover, working Monday, Thursday and Friday. We are a welcoming practice in Grimsby aiming to provide high quality care in a supportive environment.
We are looking for a committed, hardworking nurse with the enthusiasm to learn new skills and adapt to changing circumstances. Experience in general practice is preferred with competencies in immunisation and vaccinations, complex wound care and a minimum of one chronic disease management. Additional skills of cytology, family planning and management of additional long term conditions will be advantageous.
The successful applicant will take a share of the workload across our modern and well equipped purpose built sites. The role will include chronic disease management, cytology, treating minor illnesses, dressings, family planning, immunisations, vaccinations and health promotion. Main duties of the job
The post holder is responsible for the delivery of practice nursing care to the practice population. They will deliver care within the boundaries of their role, focusing upon chronic disease management, supporting GPs in care planning and monitoring of patients at risk of unplanned hospital admissions, supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities.
You will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership to other staff as required.

Pelham Medical Group's clinical team consists of GPs, Pharamcist, ANP, ACPs, Physician Associate, Practice Nurses and Senior Healthcare Assistants.
Our Reception, Administration and Management teams make up the non-clinical side of the practice.

Specific Core duties include:
Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-beingImplement and evaluate individual treatment plans for patients with a known long-term condition Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition Management of patients with diabetes and other chronic diseasesPrioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care Support the practice in the delivery of care planning and monitoring of patients at risk of unplanned hospital admissions, including following up those patients when discharged from in-patient care of A&E attendancesDeliver opportunistic health promotion using opportunities such as new-patient medicals Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions in accordance with Practice guidelinesSupport patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care Assess and care for patients presenting with wounds Support and advise women requesting information relating to family planning needs Support and manage health needs of women presenting for cervical cytology consultations Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children Advise, support and, where appropriate, administer vaccinations for patients travelling abroad Promote and deliver evidence-based care for patients presenting with aural conditions.The post holder is required to provide services to the Practices housebound patients in their own homes to ensure equitable access to primary care services.

The nurse led services involve assessment of patients, advice regarding treatment and therapies and referral to other agencies as appropriate. It is crucial that you have a good knowledge base in order to manage these patients.

This job description reflects the current main organisational priorities for the post. In the context of change taking place within the NHS, these priorities will develop and change in consultation with the post-holder in line with service business needs and priorities.
You will be required to work additional hours, according to rota allocation, up to 8.00pm Monday to Friday and on a Saturday, Sunday or bank holidays between the hours of 9am and 5pm. You are required to provide a reasonable amount of holiday and sickness cover for colleagues to meet the needs of the business. Any additional hours will be paid at single time Monday to Friday and at time and a half on Saturday and Sunday.
