Remote Monitoring Hub Support Worker
Band 4
Our vision is for the SWL service users to experience well-co-ordinated care and support in the most appropriate setting, which is truly personcentred and helps them to maintain their independence.
The post holder will require moderate assessment skills and have the ability to monitor and support patients via the Remote Monitoring/Virtual Ward with complex needs through an acute episode in the community in order to avoid a hospital admission.
The candidates will work closely with the local community teams and ensure early escalation of unwell patients. They will need to be motivated, innovative with strong leadership and communication skills, to support the development of the Remote Monitoring Hub.

Main duties of the job
The post holder will provide a high standard, efficient, cost-effective Remote monitoring/Virtual Ward service to patients with a range of acuteand chronic conditions hosted by Croydon Health Services NHS Trust in partnership with acute and community health service providers inKingston and Richmond, Merton and Wandsworth, and Sutton.
Patients with a range of acute and chronic conditions will be remotely monitored. This is to support patient discharges from hospital, preventavoidable in-patient admissions and reduce pressure on inpatient bed capacity whilst maintaining standards of care that are equivalent to those in hospital. The post holder will act as a support for the Remote Monitoring Hub/Virtual Ward team members in monitoring patients who are under the care of the Virtual Ward.
The post holder will be line managed by, and will work closely with, the Remote Monitoring team to ensure that there is an effective clinical service for the Virtual Ward Service.

Associate Nurse registered with the NMC/Awaiting GMC registration Essential
Registered Nursing Associate on the NMC register: sufficient experience or Overseas qualified Medical Doctor awaiting/working towards GMC registration. Educated to a minimum level 5 Foundation degree level specific to nursing associate practice
