Job summary
As part of the new GP Reimbursement Scheme, an exciting opportunity has arisen to join our multidisciplinary team working across Huyton and Halewood. We are a dynamic, forward thinking PCN who are passionate about developing and delivering excellent local services to meet the needs of our patient population. With the changes in funding to practices for additional General Practitioners we are looking to expand our team to employ a GP to work alongside our practices and PCN team.
Main duties of the job
To perform as an autonomous practitioner working independently with other healthcare professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting.
Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
To be professionally accountable and responsible for all elements of clinical practise in accordance with the GMC code of conduct ensuring the safety of patients and the quality of care delivered.
To work innovatively with PCN and practice colleagues around such as population health management, clinical governance, service and workforce redesign, digital care and safeguarding.

We have a well-developed, multidisciplinary team including GPs, Clinical Pharmacists, Pharmacy Technicians, First Contact Physiotherapists, Social Prescribers and Mental Health Practitioners.
Job description Job responsibilities
The postholder will undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with paperwork and correspondence in a timely fashion.
Clincially examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly, referring patients to other services/agencies in a timely manner using practice, PCN and local guidelines.
Recognise the need for and be able to perform and interpret investigatory procedures including laboratory and radiology.
Prioritise health problems and be able intervene appropriately to assist the patient in complex, urgent or emergency situations.
Ensure appropriate follow up of patients.
Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assess the health care needs of patients with undifferentiated and undiagnosed problems.
Screen patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current practice disease management protocols, develop care plans for health.
Admit or discharge patients to and from the caseload and refer to other care providers, as appropriate.
Record clear and contemporaneous consultation notes to agreed standards.
Collect data for audit purposes.
Be involved in the provision of education, supervision and training to GP Registrars and other staff.
Compile and issue computer-generated acute and repeat prescriptions, and prescribe in accordance with the practice prescribing forumlary (or generically) whenever this is clinically appropriate.
Be aware of your duties and responsibilities regarding current legislation and adhere to policies and procedures on Safeguarding Children and Adults.
Undertake home visits when required, including those of an urgent nature.
