About this role
The Social Determinants of Health (SDOH) Screener is responsible for screening applicants' social determinants. The Screener will complete NY1115 - AHC Screening, eligibility assessments, confirm Medicaid eligible, and document from standardized SDOH screenings that have been completed. The SDOH Screener will complete follow-up activities for Medicaid members in accordance with the New York State Department of Health (DOH) requirements under the 1115 Medicaid Waiver. This role supports the identification and resolution of Health-Related Social Needs (HRSN) through coordinated service engagement, referral tracking, and documentation to ensure compliance, data accuracy, and audit readiness. Flexible schedule for Full-Time, Part-Time, or Per Diem.
