The Social Worker, Care Manager is responsible for the assessment of the social, emotional and environmental needs of Care Management members, the development of participant social plans of care, negotiation of community health services required under the plans of care, and ongoing monitoring and re-assessment of Care Management members. This position will travel to locations within the VIVA HEALTH service area through a reliable means of transportation insured in accordance with Company policy.
