About this role
Empath Home Health, a member of Empath Health is seeking a compassionate Medical Social Worker MSW Home Health (PRN) to work with our Home Health team to deliver individualized care to patients in their homes in Manatee county (including Bradenton, Anna Maria Island, Lakewood Ranch, & surrounding areas). In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.
