About this role
Thresholds is seeking a compassionate and highly organized Team Leader to support individuals served through our Flexible Housing Pool (FHP) program—people navigating housing instability alongside complex health and social needs. The FHP program serves individuals experiencing or at risk of homelessness, many of whom face overlapping challenges such as chronic medical conditions, behavioral health needs, substance use, or limited access to income and community supports. While not all members have a diagnosed mental illness, many require thoughtful coordination across multiple systems to remain stable. At its core, this role is about helping people obtain and maintain safe, stable housing—and using that housing as a foundation for improved health and long-term stability.
