As a Home Care Coordinator for PACEKC, you will be part of a team of caregivers to provide care for the people who need it most. Our Home Care Coordinator oversees the day-to-day operation of the Home Care Program to ensure compliance and quality. While ensuring the optimal level of independence for our participants. As a member of interdisciplinary team, you will work closely with the clinical staff to establish a home care plan of care to meet participants' needs.
