About this role
The NP will work with our Palliative Medical Partners group providing consultation and follow-up visits for patients within our community hospital and specialty hospital partners in New Jersey. This is a full time position, where as the Nurse Practitioner will collaborate with patients, families, and their primary care and consulting physicians to provide excellent patient and family centered care. The NP will develop and maintain internal and external relationships, while meeting the needs of our patients and their families. Master’s degree in nursing; License/Certification: RN, APN license in NJ; experience in palliative care, oncology, heart failure, hospice or geriatric care is preferred but not a requirement.
