About this role
Provide case management/care coordination and advocacy by carrying a caseload and assisting clients in negotiating social services, providing follow-up and monitoring support. Reassess client need as appropriate.
Responsibilities:
• Perform client needs assessment, based on intake materials and client interview
• Meet weekly with clients enrolled in vocational training programs, to assess their level of satisfaction with the program
• Assist residents with job applications and applications to vocational and educational training programs as needed
• Meet weekly with all clients on caseload to address case management issues and concerns including budgeting, entitlement, housing and employment
• Finalize plans for referrals for therapeutic support
Skills:
• Expertise in case management and client advocacy
• Ability to work with addicted women with critical and emergent situations
• Sensitivity to cultural and economic differences
• Knowledge of regional and service pool
Experience:
• Thorough understanding of medical and psychosocial issues related to addiction
Education Requirement:
• Bachelor's Degree and 3 years experiences in social/human services or Master's degree and one year of related experience
Salary:
• Grade 15
