About this role
PHMC serves as both a direct service provider to individuals, families, and communities across the region and an intermediary agent, managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives that require operational sophistication, strategic leadership, and deep mission alignment.
RESPONSIBILITIES:
• Assist Psychiatric Nurse Practitioner with daily activities related to the health needs and well-being of the participants of Pathways to Recovery:
• Work with Admissions Manager to ensure all paperwork and UDS intake is completed within 24 hours of admission.
• Triage residents presenting health-related complaints.
• Take routine vital signs.
• Ensure all staff are trained in CPR/First Aid.
• Complete nursing assessment for all new admissions.
• Maintain NP schedule.
• Ensure all consumers receive their meds.
• Ensure all residents obtain a PE.
• Refer to PCP for all outstanding medical needs.
• Ensure prescriptions are up to date for MATs.
• Maintain consumer medical file.
• Obtain VS as needed for MATs.
• Provide health education classes for consumers.
• Ensure consumers are provided with education on STDs ( Hepatitis, HIV) and all communicable diseases.
• Ensure medical file is audit-ready at all times.
• Follow up with consumers on medication compliance.
• Assist with transport as needed.
• Assist NP with inductions.
• Assist with ordering and management of prescription medications and coordination with pharmacy.
• Assist with clerical responsibilities including answering phones and logging in mail.
• Provide coverage to AA as needed.
QUALIFICATIONS:
Experience:
• At least 2-3 years in a community or residential setting.
• Pediatric experience considered a plus.
• Experience in working with culturally diverse populations.
Education:
• Graduate of an accredited Medical Assistant school and having passed the State certification examination.
• Medical Assistant Certification.
