About this role
**Hourly rate varies based on the Self‑Direction client and the unique needs of each case.** As a Self-Directed Direct Support Professional, you will work one-on-one with individuals to help them achieve the personal goals outlined in their service plan. You’ll play an important role in fostering independence, community inclusion, and person‑centered support. Depending on the service plan, you may provide support in the individual’s home, in community settings, or both. This position requires working independently while collaborating with the individual, their broker, and their circle of support. All duties must comply with Agency, Program, and NYS OPWDD guidelines.
