About this role
As a Care Coordinator, you will be providing a single point of contact for individuals in your caseload, guiding them as they navigate a care plan, minimizing confusion in both mental and physical health arenas, and helping bridge the gap in understanding where physical and mental health intersect. A Care Coordinator will be able to help their clients mitigate the negative impacts of identified social determinants to health and wellness and improve the security of basic needs of well-being and healthy living while providing access to a litany of services to improve a client's physiological and psychological condition.
