The Transitional Services Supervisor is responsible for overseeing the re-entry services provided to members and for the direct management and support of Transitional Services Re-Entry Specialists and Peer Coaches. This role performs a broad range of member support, team leadership, and operational responsibilities, while partnering with department leadership and cross-functional teams to promote quality services, staff engagement, compliance, and effective care delivery.
