Role: supporting participants, conducting assessments, coordinating services Requirements: Provide housing navigation and case management, conduct assessments, develop strength-based action plans, coordinate wraparound services, document in HMIS/Credible/Smartsheets, and maintain current Oregon driver’s license and insurance. QMHA or PWS preferred; HMIS experience a plus.
