About this role
The Intake Coordinator works as part of a multidisciplinary team serving community-based patients within the Mosaic Primary Care Network. The Intake Coordinator reports directly to the Supervisor, Mental Health and plays a key role in creating a seamless and supportive experience for patients being referred to Mosaic for mental health services. Serving as the first point of contact, this position guides patients through an established clinical intake process, gathers essential information, and addresses questions to ensure clarity and understanding. The role is responsible for collecting and documenting patient data while maintaining strict confidentiality and compliance with organizational and regulatory policy.
