The Children’s Long Term Support (CLTS) Waiver is a federally funded Medicaid program serving the needs of children who have a developmental, physical, or a severe emotional disability. CLTS Care Coordinators serves as the main point of contact in helping families obtain CLTS services necessary to help keep their children safely in a home/community setting. This is a continuous posting in anticipation of future growth opportunities in the Waukesha County area. The position would be full time, Monday through Friday, first shift, and requires flexibility to meet the needs of the families. There is an opportunity to work remotely in this role!
