($500 Sign-On Bonus)

The Job Coach is responsible for supporting one or more individuals during their day-to-day activities in Linwood's Supported Employment Program. The Job Coach is directly accountable to the Supported Employment Coordinator.

Essential Duties and Responsibilities:

• Participates in developing the IP and Behavior Plans, implement them, and document the portion of the plans related to the day program.

• Guides one or more individuals at the job site to ensure the job duties meet the employer's expectations.

• Involved in performing the job duties along with the individual(s) as needed to maintain the production level determined by the employer.

• Guides one or more individuals at the job site to ensure successful participation in appropriate activities of daily living, including the development of self-help skills, leisure skills, and functional academic or vocational skills.

• Actively supports the individual in achieving success and integration in the community and at the work site.

• Responsible for providing transportation to assigned individuals or groups.

• Completes all required documentation in an efficient and timely manner.

• Be a role model to the individuals you serve and, at times, a representation of Linwood.

• If requested/directed, timely attendance and participation in meetings, including but not limited to staff meetings, clinical meetings, and medication reviews.

• Uses technology for the completion of specified job duties.

• Learns and properly utilizes Linwood's Data Management System (CIMS) to maintain accurate records and permit accurate billing with LTSS/DDA.

• Attends work regularly according to the assigned work schedule and per Linwood's policy.

• Attends and participates in all mandated DDA training, staff meetings, and other activities to facilitate professional development.

• Maintains current valid CPR/ First Aid and MANDT certifications.

• Works cooperatively with others, including all staff, supervisors, administrators, co-workers, people served, community professionals, customers, vendors, and the public.

• May assume other duties, responsibilities, and special projects as needed.

Minimum Qualifications (Education, Experience, Skills)

• Must be at least 21 and have a high school diploma or equivalent.

• Experience working in creating community involvement and volunteer or employment opportunities with persons with developmental disabilities preferred.

• Must have a valid driver's MD license and an acceptable driving record as determined by criteria established by the Agency's insurance carrier and by Agency policy.

• To demonstrate interest and ability to address the developmental and social-emotional needs of adults with developmental disabilities.

• To be energetic, enthusiastic, patient, and understanding.

• To be flexible enough to work cooperatively with other staff in addressing individuals' physical, social-emotional, and behavioral needs.

• To demonstrate an understanding of fundamental rights and self-determination and assure that the individuals to whom he/she provides support be afforded these rights.

• To demonstrate a comprehensive understanding of Linwood's philosophy and the ability to teach Linwood's methods and philosophy to others.

• To engage professionally with Linwood staff, parents, school system personnel, and other professionals.

• The ability to organize, document, and meet deadlines.

• To follow written and oral instructions and procedures.

Physical Demands and Work Environment

• The noise level in the work environment is usually moderate to loud.

• Regularly exposed to weather conditions during travel to and from worksites and appointments.

• Frequently required to use hands or fingers, handle, or feel objects, tools, and various technology or controls.

• Must be able to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch, run or crawl.

• Must be able to work under regular, moderate levels of stress.

• Must be able to regularly lift and move up to 10 lbs. and occasionally lift and/or move up to 50 pounds.

Shift: Monday - Friday, 8am-4pm

Pay Rate: $18.36 Per Hour
