About this role
The Adult Day Health Care (ADHC) Program at Housing Works is a unique community-based health care program that provides comprehensive nursing, social work, nutrition, mental health, and substance use services to individuals living with chronic illnesses including HIV, behavioral health conditions, diabetes, hypertension and others. Housing Works is seeking a Licensed Master Social Worker/Case Manager to provide mental health/substance use/supportive counseling, crisis interventions, and case management services to clients of the Adult Day Health Care Program, located in Brooklyn.
