Horizon is looking for a Care Coordinator, based in our location at 1000 Young St, Tonawanda, NY. The Care Coordination Team works throughout the WNY area assisting individuals who are living with mental illness, substance abuse and chronic medical conditions. We ensure that these individuals are getting the services in the community they need to obtain good physical and behavioral health! This position serves the Erie County community. This position is full-time, Monday-Friday 8:00 a.m-5:00 p.m.
